ONLINE REGISTRATION FORM
Holiday Gifts for Kids helps local families who are struggling financially give their children a gift this season. Families choose one new toy for each child, and every family takes home a board game to enjoy together.

HOW IT WORKS
1. Register by Monday, Dec. 7. Pre-registration is required. Fill out the registration form below.
2. Watch for a reminder email before the event. Once you register, you are all set to attend. We will contact you if you do not qualify. The only disqualifications would be if your address is outside of our coverage area, your child is over 17, or someone from your household has already applied.
3. Come anytime during the event. New toys are added throughout the event, so every family has a great selection. Be sure to bring the three items listed below.
WHO CAN REGISTER
• Parents or legal guardians of children ages newborn to 17 who live in ISD31, Beltrami, Clearwater or Hubbard County.
• One form per family. You may only register your own children, not friends or relatives. If you share custody, decide which parent will register.
• Families attend one event, based on their home address. Need a different location? Call us at 218.444.8929.
BRING THESE THREE ITEMS TO THE EVENT
☐ Photo ID for the parent or guardian named on the form. If someone else is picking up, they must show their photo ID and have a signed note naming them by the parent or guardian who registered.
☐ A paper or virtual document for each child showing their name and date of birth, such as a birth certificate, school, or health records. Sanford Health MyChart works well for this. Without it, we cannot give a toy for that child.
☐ Proof of physical address that matches the registration form, like a utility bill, phone bill or driver’s license. If your mail goes to a PO Box, bring something that shows your physical address.
We require these items to ensure fairness.
