Housing Trust Fund Program Application

The United Way of Bemidji Area, in partnership with Beltrami County, is soliciting proposals from qualified organizations to address the need for housing solutions within our community. This Request for Proposals (RFP) aims to support programs that enhance housing availability, affordability, and accessibility for individuals and families in Beltrami County. The Housing Trust Fund Cabinet reserves the right to revise the RFP, as needed. For questions, contact Halen Groenke at halen@unitedwaybemidji.org. Time-sensitive questions should be asked at least 10 days before a submission deadline to give time for the Cabinet to answer.

Organization Background:

Provide information about your organization, including its mission, history, and experience working on similar programs. Describe your organization’s qualifications and experience in housing programs or services related to affordable housing, if applicable.

Program Summary:

Provide a brief summary of the program, including its goals and objectives.

Program Description:

In a few paragraphs detail the program's activities, statement of need, and anticipated outcomes. Include information on how the program will address housing needs within Beltrami County and benefit the community. Include steps you will take to achieve the programs goals and objectives.

Budget:

Upload a detailed board approved budget for the housing program, including expenses such as program staff salaries, and any other relevant costs. Be sure to include both the total program cost and the amount requested from the Beltrami County Housing Trust Fund. Include other sources of funding for the housing program.

Program Evaluation: Explain how you will evaluate your program's success.

Indicators: List the measurable indicators that will demonstrate progress towards achieving each objective. Examples of measurable indicators could include criteria used to measure program success and what standards must be reached for the program to be considered successful.

Data Collection Methods: Describe the methods you will use to collect data on each indicator.

Data Analysis: Explain how you will analyze the data collected to assess program success.

Partnerships and Collaborations:

Describe any partnerships or collaborations that your organization has established for this program, including the roles and contributions of each partner.

Sustainability:

Explain how the program will be sustainable beyond the grant period, including plans for ongoing funding, success, and community support. Explain what would happen if you did not receive funds for the Beltrami County Housing Trust.

Community Impact:

Discuss the potential impact of the program on the Beltrami County area, including how it will improve access to affordable housing and contribute to overall community development. Include how many people this program will impact.

Additional Information:

Include most recent financial statements and audit. Include/Attach any supporting documents, such as program plans or letters of support (required if stated above).
